初診記錄單：初診必填，裡面包括您個人資料還有您為什麼毛病來看病，可用中文填寫。這是法律要求必須存檔。醫易堂保證此資料不會外泄給第三方。來本診所就診的患者可不用填，來本所後當場再填即可。您填完後可掃描寄至：yiyiherbs@gmail.com。

中國病患可用這份中文的初診記錄，中國以外的病患只接受英文版的初診記錄。填寫初診記錄請務必詳細真實，這份文件是正式記錄，如有填寫不詳細之處，將不被接受，謝謝合作。
